Our “Chef for a Day” service is ideal for clients returning from a hospital stay or who simply enjoy the luxury of gourmet meals specially prepared by a Personal Chef.

The chef will provide an in-home consultation and present a choice of meals to suit specific tastes and dietary preferences. You can get more about daily food group targets on https://www.nutrition.gov/

Personal Chef

The chef will shop for groceries, prepare a weeks’ worth of selected meals in the client’s own home, using the chef’s own culinary cookware, and package, label, and freeze meals in microwave-safe containers.

The Home Team’s experienced, professional care managers will conduct a thorough, in-depth assessment of an individual’s particular needs. We evaluate the client’s living situation, medical, cognitive, and emotional status, existing support systems, environment, and financial situation for remodeling the kitchen by Kitchen Remodeling Wellington. In addition to geriatric assessments, we conduct evaluations for individuals with disabilities who need support services in order to live safely at home.

Our Care Managers review options, develop a plan of care and provide written reports of their findings.

In Home Facility Assessment

Assessments include:

Review of medical records as needed, and consultation with physicians and other healthcare providers Consultation with family members and significant others Evaluation of the home or facility for safety and appropriate living situation Written recommendations and plan of care

What is the North Central Aging and Disability Partnership?

The North Central Aging and Disability Partnership (NCADP) is a partnership among Connecticut Community Care, Inc. (CCCI), North Central Area Agency on Aging (NCAA), and Independence Unlimited, Inc. (IU).  The NCADP was established with the aim of building a point-of-access and comprehensive system for long-term care information, options and supports for the North Central Connecticut community. The collaboration became effective March 1, 2009 by the authorized signing of a Memorandum of Understanding.

North Central Aging and Disability Partnership (NCADP)

NCADP offers the general public unbiased information and assistance on issues that affect older adults, people with disabilities and their families. People can call any three of the agencies involved – CCCI, IU or NCAAA – and be put in touch with trained individuals who can best answer questions they may have about services, options, financial support, etc.
