"We are very pleased to have found a care manager from Care Management Associates. She's someone who will be a local point of contact for the family, who will coordinate communication between the aides, the assisted living caregivers, and the doctors. She will make sure that someone goes to Mom's doctor appointments with her and will be there at the hospital if Mom has another emergency, keeping track of her medical history and doctor's instructions, and speaking up for her within the meat grinder of our medical care system.
